I didn't do my fellowship at Cleveland Clinic, but I am a big believer in workhorse, non-supervisory, do-the-goddamn-cases-yourself, cardiac anesthesiology fellowships (as opposed to echo fellowships). You should consider the work life balance. The work life balance even as an attending surgeon is not going to be as good as an attending anesthesiologist. The hours are still longer and is demanding. I wouldn't put too much thought into the years required for residency. Even in anesthesia, you'll need intern year plus CA1-3.

As far as Anesthesiology vs. EM, I would say EM has better "lifestyle" b/c they work less hours (probably an average of 35 vs. an average of 50 or so) and they are never on call (barring mass casualty or you have to cover for a sick co-worker).
